Job Description:
The Access to Care Supervisor is responsible for assisting in the hiring process, orientation and evaluation of all department staff. Ensures that all access to care staff manages referrals appropriately and that timely and consistent communication with sites is maintained at all times. Participates in managing referral requests including entering required information into the Electronic Medical Record, coordinating initial care with referral sources and locations. Oversees weekday, weekend and after hours staff. Utilizes and promotes use of excellent customer service skills when working with referral sources and site staff. The Access to Care Supervisor must be knowledgeable in day-to-day call center operations which includes call center system technology. The Access to Care Supervisor must be able to drive results and meet established goals while fostering an environment of teamwork and cohesion.
Job Requirements:
- A Bachelors Degree in Business Administration or related field or equivalent in education and experience is required.
- At least 2 years of Hospice, Home Health or community health experience in the last 5 years strongly preferred. Previous supervisory experience required. Demonstrated practice in project management, human resource management, and the direction of administrative services and operations.
- Must have exceptional interpersonal and customer services skills; must be able to effectively solve unique problems as they arise or identify when to consult the Access to Care Manager. Must have excellent organizational and management skills to handle multiple level complex staff and customer issues. Requires the ability to be a strong and consistent mentor to staff and use the disciplinary process appropriately. Must have knowledge of current Hospice and Home Health, AIM admission criteria, Medicare, Medi-Cal, State and Federal regulations, demonstrated functional knowledge of DME as used in the Hospice/Home Health/AIM setting
- Possesses the ability to establish and maintain effective working relationships with staff members, physicians, referral sources, patients and their families. Demonstrated analytical skills, ability to influence, group processes, and facilitation and consulting skills required. Excellent verbal/written communication skills required. Manages and performs projects with high degree of autonomy. Ability to organize, adapt and re-prioritize to meet deadlines in an environment of rapid change and system growth all required. Working knowledge and proficiency on computer software for analysis and presentation required. flexibility and independence required to work competently and autonomously in the absence of the Director of Support Services.
